HIV Program Case Manager

1 – Full-time Monday - Friday 8AM-5PM

Successful candidate must have solid experience & advanced knowledge of:

Bachelor of Science Degree in Nursing, Associate Nursing Degree.

Current RN or LPN licensure in the State of Iowa as appropriate to degree.

Current CPR and Mandatory Reporter certifications.

Basic computer and data entry skills.

Considerable knowledge of HIV and community resources and services.

Two years experience in the field of HIV care or highly motivated, self-directed.

Ability to work within the broad framework of health programs with many types of people, including health professionals and laypeople.

Knowledge and experience working with the community.

Experience relating to the needs of the patient population.

Ability to handle confidential information appropriately.

Successful candidate must perform primary functions of the position:

Assist in the overall operation of the early intervention program.

Provide nursing case management, follow-up on referrals, monitor routine lab tests and immunization schedules.

Establish partnerships with medical practices and urgent care centers in urban and rural areas to identify PLWHA.

Collaborate with the Iowa Department of Public Health and local Disease Prevention Specialist.

Provide outreach to special populations, including Hispanic and African populations, for HIV education and testing.

Work flexible hours, including weekends and evenings.

Conduct outreach to medical practices, urgent care centers, county health departments, and other agencies for collaboration.

Support and empower clients throughout the healthcare continuum with counseling, problem-solving barriers to care, and coaching on treatment adherence.

Perform nursing assessments, medical triage, and refer or monitor medical issues.

Make reminder calls, locate patients out of care, and send text reminders.

Provide HIV education to clients, families, partners, staff, and external agencies.

Manage adherence programs using evidence-based strategies and innovative modalities.

Prepare for patient appointments and assist the HIV provider team.

Perform HIV testing and manage the PrEP program, including education and appointment facilitation.

Maintain the RW Careware Database and electronic medical records accurately.

Perform other duties and projects as assigned.
