Overview
The Health Care Case Manager supports the Department of Children and Families/Child Protection and Permanency in meeting the health care and protection needs of children in out‑of‑home placement. The role collaborates with the Child Health Unit, medical homes, community service providers, and child welfare staff to ensure comprehensive health care and continuity of care for children and caregivers.
Responsibilities
Assist with enrollment of a child in a managed care entity and identify a medical home at the time a child enters out‑of‑home placement.
Retrieve and review all medical records, interview past providers and caregivers, and forward information to the medical home and other health providers.
Schedule an Initial Physical Health Evaluation, review preliminary and final reports, and ensure all recommendations for health care are completed.
Develop a child health care plan in collaboration with the child’s medical home, involved specialists, caregivers, other community service providers, and when appropriate DCF/CP&P.
Link the child and caregiver with appropriate health resources and community services.
Ensure that the child is scheduled for and attends timely EPSDT visits and specialty appointments, review all reports, and forward necessary reports to the medical home.
Qualifications
Current licensure as a Registered Nurse in the State of New Jersey.
Bachelor’s Degree in Nursing or a BA/BS degree in a health‑related field; may substitute years of pediatric nursing experience for degree.
Minimum of 1–2 years of full‑time clinical experience as a pediatric nurse in pediatric environments such as pediatric doctor’s offices, pediatric hospital units, pediatric public health nursing, pediatric emergency department/pediatric critical care unit, family practice offices, school nurse, child welfare nursing, pediatric/juvenile mental/behavioral health facilities, or children’s hospital.
Current certification in American Heart Association Basic Life Support for Healthcare Providers.
Vehicle and valid New Jersey license required for off‑site visits.
Preferred Qualifications
Two (2) years of experience in nursing case management, community health nursing, or nursing care of vulnerable families.
Physical Demands and Work Environment
Standing, bending, walking (up and down stairs), and ability to lift 50 pounds (infants/children records) to waist level.
