Job Overview AgeOptions offers the H.O.P.E. Program Opportunities – Case Manager role. This position supports individuals transitioning from long‑term care facilities to community living. The role is part of an interdisciplinary team delivering comprehensive support that includes life skill building, physical and emotional services, and ongoing case management.
What Will You Be Doing Create and implement a comprehensive, person‑centered service plan with each resident.
Assist residents in developing life skills, identifying services and supports, and advocating for necessary equipment, medical care, and supports.
Coordinate discharge planning and transition to community with client, nursing facility staff, insurance health plan, HOPE nurse, OT, and family/informal supports.
Provide ongoing comprehensive care management after the client transitions to the community, coordinating benefits, in‑home services, and community resources; schedule medical appointments; and advocate as needed.
Use motivational interviewing to create harm reduction plans, crisis plans, and 24‑hour backup plans as necessary.
Collaborate with partner agencies to facilitate all aspects of the transition.
What Knowledge and Experience Are We Looking For RN licensure (nurses).
Social worker with master's degree (MA, MSW) or LSW/LCSW/LCPC.
Minimum one year experience providing social services.
Interest in working directly with individuals during transition.
Experience with clients having severe mental health conditions, substance abuse, and/or complicated physical illness preferred.
Excellent written and oral communication skills.
Computer skills: Microsoft Office Suite; ability to learn proprietary program databases.
Knowledge of community supportive services and sensitivity to cultural diversity.
Ability to work with people who have multi‑faceted needs.
Must travel the greater Chicagoland area; daily reliable transportation required.
Experience with Community Care Program (CCP) or other care management program preferred.
Salary and Benefits Starting salary range: $50,000–$65,000 annually. Salaries are based on experience, education, licensing, and internal equity.
AgeOptions contributes 5% to your 403(b) regardless of your contribution.
Full‑time role includes participation in medical, dental, and vision plans. Hybrid environment: weekly field work and remote work.
35‑hour work week (average, Monday‑Friday).
