Overview Geriatric & Palliative Care - Advanced Practice Provider – Telehealth. Remote position; must live in Tennessee with an active TN license. Full-Time, Monday–Friday with on-call rotation.
Are you a compassionate and dedicated Advanced Practice Provider with a passion for providing exceptional care to geriatric and palliative care patients? We are seeking a skilled provider to join our team and coordinate comprehensive care in the community via telehealth. Responsibilities Under the direction of the designated supervising physician, diagnose medical conditions, plan and implement interventions, and evaluate patient response.
Assemble and document a complete patient medical database including health history and physical examination.
Document assessments, orders, interventions, results, and planning in the medical record.
Communicate findings with the supervising physician, external medical teams, and interdisciplinary team (IDT) members as appropriate.
Provide information, education, and support to patients and families; facilitate communication between patients, family members, and the IDT.
Foster teamwork and serve as a resource within the IDT; promote economical and effective primary geriatric–palliative care.
Assess eligibility for hospice services; participate in team meetings as required.
Potential to mentor and precept nursing students, medical students, medical residents and fellows; assist in orienting new providers to the team as needed.
Maintain all state and national certifications (discipline-specific), including active and unrestricted DEA; demonstrate continued competency in patient care.
Participate in required continuing education to maintain credentialing and licensure; stay current with research findings and integrate into practice.
Other duties may be assigned.
Qualifications Education and/or Experience: Graduation from an accredited APP degree program (e.g., NP, CNS, PA); master’s degree preferred. Experience in geriatrics, hospice or palliative care preferred. Experience in community-based chronic disease management preferred.
Certificates, Licenses, Registrations: Current/Active and unrestricted Tennessee licensure; maintenance of national certification in the associated discipline (e.g., NP/CNS/PA) and ability to maintain an unrestricted DEA for prescribing; if required to drive, current driver’s license and automobile insurance as required by Tennessee law.
