We are seeking a compassionate RN to join ComForCare Home Care in Lake County, Illinois as an RN Case Manager. We offer flexible scheduling and the opportunity to help clients live independently at home.
What You'll Be Doing Become a Health Detective: conduct in-depth assessments to uncover the unique needs of each client.
Be a Master Planner: create personalized care plans using clinical expertise to help clients live their best lives.
Make a Real Impact: support clients in safely managing medications and maintaining independence at home.
Join the Team: collaborate with other team members to build a support network for every client.
What We\'re Looking For Graduate of an accredited nursing program.
Current Illinois Registered Nurse (RN) license.
Minimum of one year of RN experience.
CPR-certified and proficient in safety and infection control.
