Full-Time Hospice Registered Nurse (RN) Case Manager
As a Hospice RN Case Manager at Holy Cross Home Care & Hospice, you’ll deliver one-on-one, high-quality care to patients in the comfort of their homes. You’ll assess, plan, and manage individualized care that promotes healing and independence. You will be responsible for case managing a team of 15-20 hospice patients. The RN Case Manager will collaborate with the interdisciplinary team and attend weekly Interdisciplinary meetings. You will interact with the primary care physician and/or hospice medical director as needed to effectively manage patient symptoms. This is a full-time role, primarily Monday–Friday with occasional weekend coverage when needed.
Responsibilities
Manage a caseload of 15-20 hospice patients in the home setting.
Assess patient needs, develop individualized care plans, and monitor outcomes.
Collaborate with an interdisciplinary team and participate in weekly Interdisciplinary meetings.
Communicate with primary care physicians and/or hospice medical directors to manage symptoms and ensure quality care.
Provide patient and family education and support; coordinate documentation in the EMR.
Qualifications
Graduate of an approved nursing education program.
Active RN license in the State of Maryland.
1+ years of clinical nursing experience (Hospice preferred).
Strong communication, assessment, and organizational skills.
Compassionate, dependable, and mission-minded.
Must have a current driver’s license and reliable transportation to and from work.

Pay Range: $36.70-58.71 per hour.
