Field Human Service Case Manager Compensation

• The pay for this position is 34.13 per hour
• Benefits are available to full-time employees after 90 days of employment

Field Human Service Case Manager Highlights

• Location: Miami Dade County, FL (33172, 33182).
• Required Availability: Mon-Fri 8a-5pm

Field Human Service Case Manager Responsibilities

• Conducts comprehensive evaluation of Members using care management tools and information/data review Coordinates and implements assigned care plan activities and monitors care plan progress
• Conducts multidisciplinary review to achieve optimal outcomes
• Identifies and escalates quality of care issues through established channels
• Utilizes negotiation skills to secure appropriate options and services necessary to meet the member's benefits and/or healthcare needs.

Field Human Service Case Manager Requirements

• Travel up to 75% for member-facing visits and documentation.
• This position requires proficiency with computer skills, effective communication skills, and the ability to multitask in a fast-paced environment.
• Requires a High School Diploma or GED.
• Fingerprinting is required for this position.
• Position is in-person and patient-facing.
• Preferred Qualifications:
• Bilingual Spanish/EnglishAbility to multitask, prioritize and effectively adapt to a fast paced changing environmentEffective communication skills, both verbal and written
• Case Management Certification (CCM) preferred.
• Bachelor's degree required - No Nurses. Social Work degree or related field.
