The Opportunity
As a Field Care Coordinator on the High Risk team, you will bring healthcare directly to patients’ homes by facilitating telehealth visits with a provider. You will build trusted relationships with patients and families, perform clinical assessments, evaluate home safety, document medications, review provider care plans, and teach patients and families how to use technology to coordinate care with their teams. You will be a compassionate, kind, open‑minded advocate for patients, removing barriers to care and ensuring a truly exceptional patient experience. You’ll collaborate with an integrated primary‑care team and travel throughout the Seattle area to serve patients in their homes.
What You’ll Work On
Facilitate telehealth visits in a patient’s home, using technology to ensure providers have a complete view of the living situation and that the patient exam can be completed via video.
Conduct standard patient assessments and documentation to facilitate a complete visit by the treating provider in the home and to ensure patients understand their care plan.
Provide best‑in‑class telehealth set‑up services and assist with extended‑scope duties such as clinician‑guided physical exams, health status screenings, non‑blood specimen collection, spirometry, retinopathy, EKGs, and vitals.
Coordinate care with patients, families, and outside care teams (specialists, home health, etc.) as needed; connect patients to state and local community resources related to housing, transportation, food, and daily living.
Assist patients and families to fully participate in health‑care services, navigate community resources, and complete applications for eligible benefits and resources.
Participate in daily operations of the High Risk Care Team, answering incoming phone calls, responding to emails, and facilitating supports for transitions of care services.
Teach patients and family members to connect with their care teams via technology.
What You’ll Need
At least 1 year of experience as a Certified Medical Assistant OR home‑based health‑care service provider, or full‑time Emergency Medical Technician (EMT) experience.
Active Medical Assistant (MA) and Basic Life Support (BLS) certifications, or ability to obtain certification within 6 months of hire.
Strong written and verbal communication skills; Spanish speaking is a plus.
A proven track record of persisting through change, demonstrating a forward‑thinking perspective under pressure, and consistently stepping up to take action on challenges.
Excellent rapport‑building and trust‑building skills with patients or customers, delivering individualized, human‑centered care.
Clinical skills in performing basic vitals and assessments preferred.
Experience facilitating access to community resources or social service agencies preferred.
Proficiency in computer technology and video platforms, including Mac iOS and Google Suite. Experience with Zoom and simultaneous multi‑software use preferred.
Access to safe and reliable independent transportation and ability to travel house‑to‑house throughout the day; mileage reimbursed at the IRS/Federal rate.
Compensation and Benefits
