We are currently hiring an Evening Case Manager to work in our Housing Office. As the Evening Case Manager , you would be responsible for:
Facilitating the transition of homeless persons who enter the program and assisting them in maintaining permanent housing and support services.
Performing assessments.
Developing and implementing Individualized Service Plans.
Providing support and supervision to residents during evening hours.
Organizing and facilitating support groups and social and recreation activities.
Evening Case Manager Minimum Salary: $18.00 per hour
Evening Case Manager Maximum Salary: $20.00 per hour
Qualifications:
Associates Degree in Social Work.
1-3 years of experience.
Valid Driver License.
Access to a reliable, working vehicle.
