Join a team dedicated to empowering individuals and connecting them with essential services. As an Enrollment Case Manager, you’ll be the trusted guide supporting applicants through the Home and Community-Based Services (HCBS) enrollment journey—right in their communities.

We are seeking a dedicated and detail-oriented Case Manager to join our team in supporting individuals applying for waiver programs. This role involves conducting in-home intake assessments, processing applications, and facilitating eligibility determinations to ensure timely enrollment and access to services.



Compensation:
  • $20.50/hr. ($21.50/hr. for bilingual candidates)
    Paid mileage reimbursement
Schedule:
  • Monday–Friday, 8:30 AM–5:00 PM
What You’ll Do
  • Travel throughout your assigned region to meet applicants in their homes -
    • Conduct 3–4 in-person intake visits daily in applicants’ homes

    1. Process applications through COMPASS and coordinate with eligibility teams to ensure timely and accurate determinations.
    2. Provide comprehensive case management support from initial referral through program enrollment or case closure.
    3. Maintain accurate records and track applicant data using Microsoft Excel.
    4. Utilize Microsoft Word for documentation and Microsoft Teams for internal communication and collaboration.
  • Qualifications:
    1. Ability to work in various home environments and in a fast-paced setting.
    2. Comfortable adapting to frequently changing processes due to contractual requirements.
    3. Reliable, independent transportation required for daily home visits.
    4. Strong problem-solving skills, with the ability to work independently while knowing when to escalate issues to a supervisor.
    5. Ability to pivot between tasks and projects while maintaining focus and meeting deadlines.
    6. Strong time management skills and flexibility to meet evolving program needs.
    7. Availability required between 8:30 AM – 5:00 PM, Monday through Friday.
    8. Experience in case management, social services, or a related field.
    9. Proficiency in Microsoft Excel, Word, and Teams.
    10. Familiarity with COMPASS or similar eligibility systems preferred.
    11. Strong organizational, communication, and documentation skills.

What We’re Looking For
Education & Experience Requirements:
Bachelor’s degree including at least 12 credits in sociology, psychology, gerontology, social work, or another behavioral science
OR
A combination of 4 years of relevant experience and education (with 12 college-level credits in social sciences)
Relevant Experience May Include:
  • Coordinating care or support services
  • Teaching life skills
  • Facilitating therapeutic activities or social programs
(Note: Providing hands-on personal care or household tasks does not count toward qualifying experience.)
You’ll Excel If You Have:
  • 4+ years of consistent employment history
  • Previous experience in case management, social services, or hospitality
  • Excellent organizational and communication skills
  • The ability to work independently and manage multiple priorities
  • Familiarity with local healthcare networks
  • A commitment to respecting diverse backgrounds and experiences

  • No time off is permitted during the first 2 weeks of training
  • Occasional lifting (up to 25 lbs.) may be required
