Join a team dedicated to empowering individuals and connecting them with essential services. As an Enrollment Case Manager, you’ll be the trusted guide supporting applicants through the Home and Community-Based Services (HCBS) enrollment journey—right in their communities.
Compensation:
$20.50/hr. ($21.50/hr. for bilingual candidates) Paid mileage reimbursement
Schedule:
Monday–Friday, 8:30 AM–5:00 PM
What You’ll Do
Travel throughout your assigned region to meet applicants in their homes
Conduct intake visits, explain HCBS program options, and help applicants understand their rights and responsibilities
Facilitate the application process, including data entry and submission of the Compass Application
Support applicants in selecting the services and providers that best meet their needs
Build strong partnerships with local healthcare providers, community organizations, and other stakeholders
Monitor and report on case progress to ensure timely enrollments
Contribute to improving processes and removing barriers for applicants
Represent the organization with professionalism, compassion, and cultural sensitivity

What We’re Looking For
Education & Experience Requirements: Bachelor’s degree including at least 12 credits in sociology, psychology, gerontology, social work, or another behavioral science OR A combination of 4 years of relevant experience and education (with 12 college-level credits in social sciences)
Relevant Experience May Include:
Coordinating care or support services
Teaching life skills
Facilitating therapeutic activities or social programs
(Note: Providing hands-on personal care or household tasks does not count toward qualifying experience.)
You’ll Excel If You Have:
4+ years of consistent employment history
Previous experience in case management, social services, or hospitality
Excellent organizational and communication skills
The ability to work independently and manage multiple priorities
Familiarity with local healthcare networks
A commitment to respecting diverse backgrounds and experiences
Please Note:
No time off is permitted during the first 2 weeks of training
Occasional lifting (up to 25 lbs.) may be required
