Enhanced Care Management (ECM) Lead Care Manager - Santa Clara County
Base pay range: $29.00/yr - $32.00/yr

As a Lead Case Manager, you will guide members at every step, arranging services they need to thrive and building authentic, trusting relationships.

Frequent in-person visits to form strong, personal connections and act as a pivotal support system

Responsibilities

Frequent In-Person Visits to Members: Regular face-to-face assessments in members\' homes, shelters, or community centers; establish trust and address concerns

Comprehensive Care Coordination: Schedule doctor\'s appointments, organize follow-up care, link to social services, ensure full continuum of support

Case Management with a Heart: Empathetic assessments; maintain close contact via phone, video, and in-person visits; address new barriers

Resource Management: Identify, coordinate, and optimize local resources (housing assistance, job training, childcare)

Patient Advocacy: Push for timely treatments and fair access to services; resolve roadblocks

Communication: Keep members, families, healthcare teams, and community organizations aligned on care objectives

Documentation: Maintain detailed records of assessments, care plans, and progress notes

Continuous Improvement: Use data and observations to refine care strategies

Regulatory Compliance: Stay current with Medi-Cal, CalAIM, and other regulations

Professional Development: Attend trainings to sharpen skills in cultural competence, motivational interviewing, and crisis intervention

Other Duties: Collaborate with the team and support broader program outcomes

Skills & Qualifications

Residency: Must reside in Santa Clara County

Experience: 3-5 years in case management, social services, or healthcare

Expertise: Familiarity with Medi-Cal, CalAIM, and Enhanced Care Management

Healthcare Insight: Understanding of healthcare systems and local community resources

Interpersonal Skills: Strong communication, empathy, and cultural competence

Organizational Ability: Time management and attention to detail

Technical Proficiency: Experience with case management software and related tools

Pre-screen assessment required

Driver\'s License: Valid California Driver\'s License (Class C), personal vehicle, and liability insurance; pass Motor Vehicle Report (MVR) background check

Employment Type: Full-time

Schedule: 8-Hour Shift, Monday to Friday 1:30pm - 10:00pm
