Enhanced Care Management (ECM) Lead Care Manager - Santa Clara County
This role is one of the first 25 applicants in a 1‑week post period. The pay range is $29.00 – $32.00 per hour and is based on skill and experience.

At Pacific Health Group, we focus on the social determinants of health and provide community‑based services that meet each member’s needs. As a Lead Care Manager, you will create personalized care plans, guide members through all necessary services, and build authentic, trusting relationships.

Key Responsibilities
Frequent In‑Person Visits
Conduct multiple on‑site visits each month in members’ homes, shelters, or community centers.
Use visits to establish trust, gather insights, and address concerns.
Comprehensive Care Coordination
Schedule doctor’s appointments and organize follow‑up care.
Link members to social services and secure necessary resources.
Case Management with a Heart
Perform empathetic assessments beyond forms and checkboxes.
Maintain close contact by phone, video, and in‑person visits.
Resource Management
Identify and coordinate local resources such as housing assistance and childcare services.
Patient Advocacy
Champion members’ rights and resolve roadblocks that hinder progress.
Communication
Serve as the central point of contact for all stakeholders.
Documentation
Maintain meticulous records of assessments and care plans.
Continuous Improvement
Use data and observations to refine care strategies.
Regulatory Compliance
Stay current on Medi‑Cal, CalAIM, and other regulations.
Professional Development
Attend trainings in cultural competence, motivational interviewing, and crisis intervention.
Other Duties
Support the team and share best practices to strengthen overall outcomes.
Required Qualifications
Must reside in Santa Clara County.
3–5 years of experience in case management, social services, or healthcare.
Familiarity with Medi‑Cal, CalAIM, and Enhanced Care Management.
Understanding of healthcare systems and local community resources.
Strong communication, empathy, and cultural competence.
Proven time‑management skills and attention to detail.
Competence using case management software and related tools.
Successful completion of a pre‑screen assessment required.
Valid California Driver’s License (Class C minimum), vehicle for daily business use, and liability insurance meeting California minimums.
All selected candidates will pass a Motor Vehicle Report background check prior to employment.
