Base pay range: $29.00/yr - $32.00/yr

As a Lead Care Manager, you will guide members at every step, arranging the services they need to thrive and building authentic, trusting relationships along the way.

Responsibilities

Frequent In-Person Visits to Members: Regular face-to-face assessments in members’ homes, shelters, or community centers; establish trust and address concerns on-site.

Comprehensive Care Coordination: End-to-end service arrangement including scheduling doctor appointments, organizing follow-up care, linking to social services, and ensuring resources for ongoing support.

Case Management with a Heart: Empathetic assessments and continuous support via phone, video, and in-person visits; address barriers and milestones.

Resource Management: Identify and coordinate local resources (housing assistance, job training, childcare, etc.) to support overall wellbeing.

Patient Advocacy: Promote timely treatments, manage insurance authorizations, and resolve obstacles to care.

Communication: Serve as central point of contact for members, families, healthcare teams, and community organizations; ensure seamless handoffs.

Documentation: Maintain detailed records of assessments, care plans, and progress notes for transparency and accountability.

Continuous Improvement: Use data and feedback to refine care strategies and ECM program effectiveness.

Regulatory Compliance: Stay current with Medi-Cal, CalAIM, and related regulatory standards.

Professional Development: Participate in trainings and workshops to enhance skills (e.g., cultural competence, motivational interviewing, crisis intervention).

Other Duties: Support the team and share best practices to improve outcomes.

Qualifications

Residency: Must reside in Santa Clara County

Experience: 3-5 years in case management, social services, or healthcare

Expertise: Familiarity with Medi-Cal, CalAIM, and Enhanced Care Management

Healthcare Insight: Understanding of healthcare systems and local community resources

Interpersonal Skills: Strong communication, empathy, and cultural competence

Organizational Ability: Time management and attention to detail

Technical Proficiency: Experience using case management software and related tools

Pre-screen assessment: Successful completion required

Driver: Valid California Driver’s License (Class C), operable personal vehicle, and minimum liability insurance; may require Motor Vehicle Report background check

Job Details

Job Type: Full-time

Pay: $29.00 - $32.00 per hour

Schedule: 8-hour shift; Monday to Friday 1:30pm - 10:00pm

Work Location: On the road
