Overview Enhanced Care Management (ECM) Lead Care Manager - San Joaquin County. Base pay range: $27.00/hr - $30.00/hr. At Pacific Health Group, ECM programs focus on addressing social determinants of health and providing community-based services that meet individuals’ needs. As a Lead Case Manager, you’ll guide members at every step, arranging services and building authentic, trusting relationships.
Responsibilities Frequent in-person visits to members: conduct on-site assessments in homes, shelters, or community centers; establish trust; address concerns promptly.
Comprehensive care coordination: schedule doctor appointments, organize follow-up care, link to social services, and ensure full continuum of support (including home health, transportation, meals, etc.).
Case management with a heart: empathetic assessments; maintain close contact via phone, video, and in-person visits; address barriers and celebrate milestones.
Resource management: identify and coordinate local resources (housing assistance, job training, childcare) to support overall wellbeing.
Patient advocacy: push for timely treatments, insurance authorizations, and fair access; manage appeals when needed.
Communication: serve as central point of contact among members, families, healthcare teams, and community organizations; coordinate care conferences as needed.
Documentation: maintain detailed records of assessments, care plans, and progress notes; ensure transparency and accountability.
Continuous improvement: use data and observations to refine care strategies and ECM program effectiveness.
Regulatory compliance: stay current with Medi-Cal, CalAIM, and related regulations; ensure practices meet standards.
Professional development: participate in trainings to enhance cultural competence, motivational interviewing, and crisis intervention.
Other duties: support team and share best practices to improve outcomes.
Qualifications Residency: must reside in San Joaquin County
Experience: 3-5 years in case management, social services, or healthcare
Expertise: familiarity with Medi-Cal, CalAIM, and Enhanced Care Management
Healthcare insight: understanding of healthcare systems and local resources
Interpersonal skills: strong communication, empathy, and cultural competence
Organizational ability: time management and attention to detail
Technical proficiency: experience with case management software
Successful completion of a pre-screen assessment required
Driver’s license: valid California Driver's License (Class C), access to a personal vehicle, and current liability insurance; pass Motor Vehicle Report (MVR) background check

8-Hour Shift, Monday to Friday, 8:30am PST - 5:00pm PST
