Overview
At Pacific Health Group, we are a catalyst for positive change in our communities. Our Enhanced Care Management (ECM) programs address social determinants of health and provide community-based services that meet individual needs. As a Lead Case Manager, you will guide members at every step, arranging all necessary services and building authentic, trusting relationships.
Responsibilities
Frequent in-person visits to members: conduct on-site assessments in homes, shelters, or community centers; build trust; gather insights; and address concerns
Comprehensive care coordination: schedule appointments, organize follow-up care, link to social services, and ensure full continuum of support
Case management with a heart: empathetic assessments; ongoing support via phone, video, and in-person visits; monitor progress and address barriers
Resource management: identify and coordinate local resources such as housing, childcare, job training, and other community services
Patient advocacy: push for timely treatments, insurance authorizations, and fair access to services; resolve roadblocks
Communication: maintain alignment among members, families, healthcare teams, and community organizations; ensure seamless handoffs
Documentation: maintain detailed records of assessments, care plans, and progress notes
Continuous improvement: use data and observations to refine care strategies and ECM program effectiveness
Regulatory compliance: stay current with Medi-Cal, CalAIM, and related regulations to ensure legal and quality standards
Professional development: participate in trainings and workshops to enhance skills such as cultural competence and crisis intervention
Other duties: collaborate with the team and support additional tasks as needed
Qualifications
Residency: Must reside in San Diego County
Experience: 3-5 years in case management, social services, or healthcare
Expertise: Familiarity with Medi-Cal, CalAIM, and Enhanced Care Management
Healthcare insight: Understanding of healthcare systems and local community resources
Interpersonal skills: Strong communication, empathy, and cultural competence
Organizational ability: Time management and attention to detail
Technical proficiency: Experience with case management software and related tools
Pre-employment: Successful completion of a pre-screen assessment
License: Valid California Driver's License (Class C minimum) and a personal, operable vehicle with current liability insurance; must pass a Motor Vehicle Report (MVR) check
