Overview Enhanced Care Management (ECM) Lead Care Manager - San Diego County. Base pay range: $25.00/yr - $29.00/yr. Location: Hybrid remote in San Diego County, CA - On the road.
This role focuses on addressing social determinants of health and providing community-based services. As a Lead Case Manager, you'll guide members at every step, arranging all necessary services and building authentic, trusting relationships.
Responsibilities Frequent in-person visits to members in homes, shelters, or community centers to conduct assessments and establish trust
Comprehensive care coordination: schedule medical appointments, organize follow-up care, link to social services, arrange transportation, and secure housing or financial support as needed
Case management with a focus on empathy: assess backgrounds and challenges, maintain ongoing contact via phone, video, and in-person visits
Resource management: identify and coordinate local resources (housing assistance, job training, childcare) to support overall wellbeing
Patient advocacy: pursue timely treatments, insurance authorizations, and fair access to services; assist with appeals when necessary
Communication: coordinate care conferences among primary care, social work, and rehabilitation teams to align on care plans
Documentation: maintain detailed records of assessments, care plans, and progress notes
Continuous improvement: use data and observations to refine care strategies and ECM program processes
Regulatory compliance: stay current with Medi-Cal, CalAIM, and related regulations
Qualifications Residency: Must reside in San Diego County
Experience: 3-5 years in case management, social services, or healthcare
Expertise: Familiarity with Medi-Cal, CalAIM, and Enhanced Care Management
Healthcare insight: Understanding of healthcare systems and local community resources
Interpersonal skills: Strong communication, empathy, and cultural competence
Organizational ability: Time management and attention to detail
Technical proficiency: Experience with case management software
Pre-employment: Successful completion of a pre-screen assessment
Driver requirements: Valid California Driver's License (Class C), a personal vehicle, current liability insurance; may require Motor Vehicle Report (MVR) background check
