The RN Case Manager demonstrates the knowledge, critical thinking and judgement for nursing and case management practice according to the ANA Nursing Scope of Practice, and nationally recognized case management practices according to the American Case Management Association. In collaboration with other clinical disciplines, this role integrates these evidence-based practices to facilitate optimal coordination of care, patient, family and clinical staff communication and education, and optimal transition/discharge planning in accordance with CMS (Medicare's) Condition of Participation. The RN Case Manager serves as an expert in coordinating the patient care trajectory through the continuum of care and that the patient is at the right level of care that is appropriate. The Case Manager must be able to demonstrate knowledge of growth and development to coordinate patient care appropriate to the age of patients served on their assigned clinical area.
