Shift Details: Part-Time; Two (2) 12-hour shifts weekly; rotating shifts (7a-7:30p/7p-7:30a), with weekend rotations every 4th weekend.
The Emergency Department Care Coordinator collaborates with the clinical team to deliver navigation and coordination of services for patients in the Emergency Department. The role involves providing status recommendations, promoting patient wellness, improving care outcomes, and ensuring efficient utilization of health services. The Care Coordinator aims to impact metrics such as throughput, length of stay, readmissions, avoidable utilization, and denials. They serve as the Subject Matter Expert for Medicare reimbursement compliance.
Requirements and Qualifications:
Associate's Degree in Nursing required; Bachelor's preferred.
Maryland RN License.
American Heart Association CPR Certification.
Case Management Certification (CCM) within 3 years.
Knowledge equivalent to 2 years of college in a specialized field.
