Under the direction of the Program Manager, the Case Manager provides direct service, as well as coordinates care management and functions as a part of a "Care Team" for the Enhanced Care Management (ECM) Program. The Case Manager is responsible for providing coordinated care for patients to improve quality outcomes, reduce health disparities, and transform the delivery system through value-based initiatives, and modernization. Oversees provision of enhanced care management (ECM) services and implementation of the care plan. The Case Manager offers services where the member lives, seeks care, or finds most easily accessible and connects members to medical care and other social services the member may need.
