The Director of Quality Management oversees the development, implementation, and maintenance of quality management programs for Hospice and Palliative Care.
Responsibilities Develop a Quality Assurance/Performance Improvement (QAPI) Plan for Hospice consistent with regulatory requirements, industry standards, and patient needs.
Keep senior leadership abreast of trends within healthcare quality, advise and recommend strategic quality initiatives, and report on key quality indicators.
Oversee the utilization of On-Line Risk Management System (Safetrak) to identify and reduce risks.
Collaborate with leadership and hospice staff to develop clinical performance indicators, manage related analysis, benchmarking, and dissemination.
Ensure consistent development, implementation, and accountability of performance improvement initiatives.
Ensure compliance with Federal and NYS regulations by serving as a resource on regulatory matters.
Maintain knowledge of current trends and developments; attend professional organization services and continuing education.
Oversee CMS Hospice Quality Reporting Program (CAHPS and HIS) to ensure compliance with regulatory mandates and utilize data to implement improvement projects.
Apply statistical process control tools.
Qualifications Registered Professional Nurse licensed in New York State.
Bachelor of Science in Nursing.
Master of Science completed or in progress in nursing or related field.
Minimum 5 years’ experience in Quality Management/Performance Improvement.
Prior experience in hospice.
Knowledge of Federal and NYS DOH Regulations.
