Key Responsibilities

Serve as the primary clinical informatics leader, ensuring optimal utilization and performance of MatrixCare across facilities

Lead the development, implementation, and oversight of clinical systems, workflows, and documentation standards

Monitor and support ongoing regulatory compliance, survey readiness, and clinical documentation integrity

Provide expert guidance on CMS Quality Measures, including analysis, reporting, and improvement strategies

Collaborate closely with facility teams and leadership to support clinical excellence and operational success

Partner with the Chief Nursing Officer to evaluate clinical outcomes and drive continuous quality improvement initiatives

Assist facilities in achieving excellence during state and federal surveys by offering guidance, training, and preparation support

Deliver ongoing staff education and training in clinical informatics, MatrixCare navigation, and best‑practice documentation standards

Review and analyze clinical data, identify trends, and recommend corrective actions where needed

Support QAPI program development, oversight, and performance improvement activities across assigned facilities

Serve as a resource to Directors of Nursing, Regional Clinical Leaders, and interdisciplinary teams for complex clinical and regulatory matters

Participate in on‑site facility visits as needed to provide consultation, support, and system reviews

Maintain up‑to‑date knowledge of long‑term care regulations, informatics standards, and evidence‑based nursing practices

Demonstrate strong leadership, professionalism, and a commitment to exceptional resident care and staff support

Requirements

Must reside within a 40‑mile radius of Louisville, KY

Active Registered Nurse (RN) license in good standing

Demonstrated expertise with MatrixCare, clinical informatics, and regulatory standards

Prior leadership experience in long‑term care; DON or Regional Clinical role strongly preferred

Strong working knowledge of CMS Quality Measures, QAPI, and survey readiness

Ability to work in a hybrid capacity with on‑site facility support and remote flexibility

Travel as needed to support assigned facilities and leadership functions

Reports directly to the Chief Nursing Officer

Monday‑Friday, with availability for occasional after‑hours or weekend support

Excellent communication, analytical, and organizational skills
