Under the supervision of the Clinical Physician, the Diabetes Nurse Care Coordinator promotes optimal health and quality of care for inpatients with diabetes through education, clinical support, and coordinated care. The coordinator serves as a clinical leader and educator, collaborating with multidisciplinary teams to improve outcomes and enhance the patient experience.
Department Essential Functions
Collaborates with departments and care teams to ensure high-quality, patient-centered care.
Leads and supports shared decision-making initiatives that improve clinical quality and patient satisfaction.
Develops and communicates effective program policies, procedures, and educational materials.
Organizes diabetes clinics, educational sessions, and outreach activities; tracks patients and maintains data for quality and grant reporting.
Partners with dietitians, case managers, and social workers to deliver coordinated, comprehensive care.
Develops individualized care plans and ensures continuity of follow-up for patients with diabetes.
Provides education to patients, families, and staff on diabetes prevention, management, and self-care strategies.
Supports research, quality improvement, and evidence-based practice initiatives that strengthen diabetes care delivery.
Utilizes clinical informatics to streamline workflows, enhance safety, and optimize care processes.
Maintains compliance with institutional and regulatory requirements by completing all mandatory training and certifications.
EDUCATION and/or EXPERIENCE
Bachelor's Degree, or diploma in Nursing from an Accredited institution
Current Maryland licensure or eligibility to obtain Maryland licensure
Excellent customer service and communication skills
Minimum of three years clinical experience in related field is required
Certified Diabetic Educator (CDE), or be eligible to obtain national CDE certification within one year of commencing the position

The referenced base salary range represents the low and high end of University of Maryland’s Faculty Physician’s Inc. salary range for this position. Some candidates will not be eligible for the upper end of the salary range. Exact salary will ultimately depend on multiple factors, which may include the successful candidate's geographic location, skills, work experience, market conditions, internal equity, responsibility factor and span of control, education/training and other qualifications.
