Position Summary

The Crisis Care Coordinator (CCC) is responsible for managing and coordinating the delivery of crisis intervention services via the Crisis Care Team (CCT). This role supports the Rural Health Care Services Outreach Program, aimed at improving behavioral health outcomes in rural communities in Northcentral Arkansas. The CCC will ensure that individuals in crisis receive timely, compassionate, and effective care through telehealth platforms.

Key Responsibilities

  1. Coordination of Crisis Services
    • Oversee the day-to-day operations of the Crisis Care Team (CCCT).
    • Act as the first point of contact for clients, law enforcement, and healthcare providers accessing the CCT.
    • Develop, implement, and refine workflows for crisis response and follow-up care.
  2. Telehealth Platform Management
    • Manage and maintain telehealth systems used by the CCT, ensuring reliable connectivity and usability.
    • Provide basic troubleshooting and technical support for telehealth users.
  3. Crisis Intervention
    • Collaborate with behavioral health specialists to assess client needs during crises.
    • Connect clients with appropriate services, including counseling, case management, and psychiatric care.
    • Ensure safety plans are developed and implemented for individuals in crisis.
  4. Community Engagement and Outreach
    • Build relationships with local law enforcement, first responders, and community organizations to facilitate seamless referrals.
    • Promote awareness of the CCT program within the community.
    • Coordinate Mental Health First Aid and health literacy training sessions in partnership with the Behavioral Health team.
  5. Data Collection and Reporting
    • Track client encounters and outcomes, ensuring data accuracy and compliance with reporting requirements.
    • Prepare reports to evaluate the program’s impact and identify areas for improvement.
  6. Training and Development
    • Provide ongoing training for CCT team members on evidence-based practices, including Cognitive Behavioral Therapy (CBT) and Motivational Interviewing (MI).
    • Stay updated on best practices in telehealth crisis intervention and behavioral health care.
