Main Purpose of Job
The main purpose of this position is to provide case management, screening, referral and follow‑up to individuals engaged in the criminal justice system as referred by court diversion and/or MCCJTP partners. This position frequently coordinates with community referral agencies, service providers and the judiciary to meet individual needs and care in the community. The position also maintains accurate and timely records of all client contacts, correspondence and data tracking forms.

Minimum Qualifications

Education: Graduation from an accredited high school or possession of a high school equivalency certificate.

Experience: One year of professional experience in assisting in the coordination of a health‑related program(s) or referring people to governmental and private resources.

Substitutions: Candidates may substitute 30 college credit hours from an accredited college or university for the required experience or may substitute U.S. Armed Forces military service experience as a commissioned officer in Health Care Administration classifications or Civil Affairs Specialist specialty codes for the required experience and education.

Desired or Preferred Qualifications

A bachelor's degree in social work, psychology or related field.

Experience working with individuals with serious mental illness (SMI) or co‑occurring and forensic backgrounds.

Experience maintaining confidential files, logs and program data.

Strong interpersonal communication skills via telephone, written communication and computer proficiency.

Applicants who meet the minimum (and selective) qualifications will be included in further evaluation. The evaluation may be a rating of your application based on your education, training and experience as they relate to the requirements of the position. For education obtained outside the U.S., an evaluation for equivalency by a foreign credential evaluation service may be required before starting employment.
