This is a great opportunity to work with dedicated and multidisciplinary staff. The Complex Care Manager Collaborates with providers and practice staff in identifying appropriate patients for care management, utilizing high-risk registry and reports generated by insurers.
Responsibilities include:
Formulating and implementing a care management plan that addresses the patient's identified needs by assessing the patient/family needs, issues, resources, and care goals.
Educating the patient/family, promoting self-management, and empowering patients/families to achieve maximum levels of wellness and independence through coaching techniques, like motivational interviewing, through telephone calls and in-office care management visits.
Developing collaborative working relationships with insurance case managers and other key parties working with a particular patient/family case; negotiating on behalf of the patient with third parties for cost-effective, high quality services, and to maximize the efficient use of resources.
Working as primary contact with Beth Israel Deaconess Physician Organization to ensure timely case management of patients in accordance with risk-based contracts with payers.
Requirements include:
Bachelor of nursing with at least three years of experience preferably in an ambulatory setting with Family Practice focus; Care Management experience preferred.
Massachusetts Nursing license – RN, Experience with telephone assessment and crisis management; excellent communication and documentation skills.
Work schedule: Monday – Friday, with two - three sessions per week as a Triage Nurse and the equivalent to two sessions per week seeing patients for face-to-face Care Management. Excellent Compensation and benefits Package.
