Eliot's Developmental Disabilities division is seeking a Community Supports Case Manager to support individuals in enriching their lives, obtaining housing and employment, developing personal skills and competencies, and enjoying a full range of community activities while building a network of caring relationships.

Responsibilities Assist in completing assessments and Individual Service Plans (ISPs) / Plans of Care (POCs) for assigned cases in a timely manner and in accordance with DDS and MRC policies and standards.
Ensure individuals are involved in identifying and developing their own goals.
Document daily progress notes for all individuals in accordance with Eliot policy and DDS/MRC documentation and billing requirements.
Develop and maintain a positive and professional working relationship with individuals, families, and providers. This includes maintaining a set schedule with individuals, timely communication and follow up on issues and requests, and being on time for appointments, meetings, and activities.
Coordinate service delivery for assigned cases (e.g., case conferences, Section 8 Housing, SSI) and advocate for the consumer with outside agencies and providers.
Provide service linkages to other providers as necessary; identify and schedule appointments for/with individuals as needed or required.
Qualifications Bachelor’s degree or equivalent experience
Experience working with intellectually disabled or brain-injured adults
Ability to manage a client caseload in an outreach setting
Valid driver’s license and a reliable vehicle
Schedule & Location Schedule: Tuesday–Saturday, 9am–5pm. Community-based position serving the Lawrence, Lowell, Burlington, and Arlington areas. Main office located in Lexington, MA.
Compensation Hourly Rate: $20.00
