Eliot's Developmental Disabilities division is seeking a Community Supports Case Manager . This division offers support and opportunities for individuals to enrich their lives, obtain housing and employment, develop personal skills and competencies, and enjoy a full range of community activities while developing a network of caring relationships.
The ideal candidate will have experience working with adults with developmental disabilities and will have the ability to advocate for community supports and services on behalf of the individuals.
Responsibilities:
Assist in completing assessments and Individual Service Plans(ISP)/Plans of Care(POC) for assigned cases in a timely manner and as indicated by DDS and MRC policies and standards.
Ensure individuals are involved in identifying and developing their own goals.
Document daily progress notes for all individuals in accordance with Eliot Policy and DDS/MRC documentation and billing requirements.
Develop and maintain a positive and professional working relationship with individuals, families and providers. This includes maintaining a set schedule with individuals, timely communication and follow up on issues and requests and being on time for appointments, meetings and activities.
Coordinate service delivery for cases assigned i.e., case conferences, Section 8 Housing, SSI and advocate for the consumer with outside agencies and providers.
Provide service linkages to other providers as necessary. Identify and schedule appointments, for/with individuals, as needed or required.
Qualifications:
Bachelor’s degree or equivalent experience
Experience working with intellectually disabled or brain injured adults.
Ability to manage a client caseload in an outreach setting.
Valid driver’s license and a reliable vehicle.
Schedule: Tuesday-Saturday, 9am-5pm
Community based position that services the Lawrence, Lowell, Burlington and Arlington areas. Main office located in Lexington MA.
Hourly Rate: $20 - $20 USD
