Are you passionate about supporting youth and families with personal, empowering care rooted in community? MBI Health Services, LLC is seeking a committed and compassionate Wrap Care Coordinator to join our team. In this role, you will be at the heart of a collaborative process that brings families, schools, and community supports together, delivering strength-based care coordination using the evidence-based Wraparound model. As a school‑based coordinator, you will be assigned to a specific school and support youth both on campus and in the surrounding community, walking alongside families during challenging times to build sustainable plans for long‑term success and well‑being.
Essential Duties And Responsibilities
Conveniently organize and lead Child & Family Teams to develop a wraparound plan of care for each child and family.
Meet the child and family, conduct a strengths‑based inventory, convene the team, and develop a care or treatment plan based on needs, goals, and available resources.
Use life domains to assess family needs and create safety plans for each child to ensure structured supports and supervision.
Assist families in identifying and obtaining formal services through DBH to meet mental health, social, and other support needs.
Maintain a caseload of 9‑11 families, providing weekly face‑to‑face home and community visits with both youth and families.
Complete all necessary paperwork, including progress notes, care plans, and letters to the court.
Perform all other duties as assigned.
Qualifications
Master's degree preferred in social work, psychology, or a related profession.
Two years of supervisory experience.
Five years of case management experience.
Computer skills, including Microsoft Office and Google Suite.
Ability to work in a diverse environment.
Excellent oral and written communication skills.
