Role: Community Case Manager – Community Case Management (RN)
Join LMHS to directly impact the health of friends, family, and neighbors in the local community. This role focuses on chronic disease management education in patients’ homes, with a special emphasis on diabetes education.
Responsibilities Provides direct patient care
Educates patients and families on chronic disease processes
Conducts diabetes-focused education for groups and individual patients (Type 1, Type 2, Gestational Diabetes, and Pre-Diabetes)
Receives and assesses referrals from referring providers
Collaborates with the multidisciplinary team to develop, implement, and evaluate care plans
Documents patient care activities in medical records accurately and timely
Coordinates patient care with appropriate disciplines
Requests physician consultation for changes in patients’ condition
Qualifications and Requirements Registered nurse with two years of nursing experience
Clinically competent with knowledge of chronic disease management
Effective communication skills
Knowledge of policies and procedures
CPR certification
Reliable transportation, valid driver's license, and automobile insurance
Current Ohio nursing license
LMHS is accredited by DNV and TJC and may require annual education related to specialty certifications and standards
