Community Case Manager — West New York, NJ • Full-time
Position Summary
Provide brief and concise statement explaining the main purpose for the existence of this position. The Community Case Manager will work with newly diagnosed and out of care clients to connect them to care, medical case management and other supportive services, under the supervision of the Client Services Manager.
Responsibilities
Under the supervision of the Client Services Manager, perform duties relating to community case management and referral services as needed.
Work with newly diagnosed and out of care clients to connect them to care, medical case management and other supportive services.
Provide Community Based Case Management in North Hudson.
Conduct community outreach, provider education and client recruitment of newly diagnosed and out of care clients in North Hudson.
Refer clients to medical care and medical case management; conduct assessments to determine clients’ supportive service needs; attend medical and other social service appointments with clients.
Collaborate with primary care providers and MCM staff to identify newly diagnosed patients in need of community based case management.
Document baseline viral load, CD4 count, AIDS diagnosis and antiretroviral prescriptions for 100% of clients.
Major Duties & Responsibilities

Documentation of baseline viral load, CD4 count, AIDS diagnosis and antiretroviral prescriptions for 100% of clients.
Required Knowledge, Skills And Abilities: Active listening skills; Familiarity with trauma informed care approach; Knowledge of available services and community resources in North Hudson; Bi-lingual (English and Spanish)
People Management Experience: Supervisory experience not required
Preferred Qualifications
Bachelor's degree and one-year previous office experience, or additional years of relevant office systems experience in lieu of a degree
Candidate must have excellent oral presentation and writing skills
Expressed commitment to Hyacinth philosophy
Other Requirements
Candidate must have flexibility to work nights and some weekends as needed.
