TenderLove Community Center is seeking a compassionate, organized, and mission-driven Rapid Rehousing Case Manager to join our team under the Continuum of Care (CoC) Program. This individual will provide trauma-informed, client-centered case management to individuals and families enrolled in our Rapid Rehousing (RRH) program. The Case Manager will report directly to the CoC Program Director and play a critical role in helping clients transition from homelessness to stable, independent living.
Compensation: Commensurate with experience
Key Responsibilities:
Provide direct case management services to RRH clients, including individualized housing plans, needs assessments, and ongoing support.
Assist clients in locating, securing, and maintaining safe and affordable housing.
Collaborate with landlords, housing providers, and community resources to identify appropriate housing opportunities.
Conduct home visits, regular follow-ups, and reassessments to ensure client progress and program compliance.
Coordinate supportive services, including employment support, mental health referrals, and life skills development.
Ensure accurate and timely documentation in HMIS and internal databases.
Support clients in achieving long-term housing stability and self-sufficiency.
Attend regular team meetings and training to remain up to date with CoC and HUD program requirements.
Qualifications:
Bachelor’s degree in social work, Human Services, or a related field (or equivalent experience).
At least 1 year of case management or related experience, preferably with homeless or vulnerable populations.
Familiarity with HUD’s Rapid Rehousing model and trauma-informed care practices.
Strong organizational, communication, and documentation skills.
Valid driver’s license and reliable transportation for home visits.
