As a CMS Care Coordinator (RN) within the 5-2 NCHD Children's Medical Services Program at the Georgia Department of Public Health, you will join a dedicated group of public health professionals that help support the overall mission and vision of protecting the lives of Georgia residents in a variety of ways.
Job Responsibilities
Under general supervision, assess client health problems and needs, develop and implement nursing care plans, maintain medical records, administer nursing care to ill, injured, convalescent, and/or disabled clients. The Care Coordinator may also advise clients on health maintenance, disease prevention, and provide case management.
Minimum Qualifications
Associate degree in nursing from an accredited college or university and one (1) year of nursing experience and completion of a PHN course within one year of hire and current Georgia Registered Professional Nurse license and CPR certification; or one (1) year of experience at the lower-level PH Registered Nurse 1 (HCP190) or position equivalent.
Preferred Qualifications
A nurse licensed in Georgia with a minimum of one year of experience working in the delivery of services to children with special health care needs and/or women's health; and a professional (Bachelor or Master Level) with one year of human service delivery experience in health and human services. A licensed nurse who also has home visiting experience.
