Full Time, Non-Exempt
Role Overview:
Seeking candidates with Master’s credentials (MSW or MFT) and experience in homeless or supportive services for our Valley Vista PSH program. The Clinical Case Manager provides case management and supportive services to help veterans break homelessness cycles and reintegrate into the community after custody release. Responsibilities include:
Providing mobile and field-based case management in areas such as independent living, housing stabilization, benefits, community integration, employment, and healthcare linkage.
Developing and implementing Housing Stability Plans with clients.
Supporting clients in achieving and maintaining stable housing and employment.
Using evidence-based practices like Motivational Interviewing, Harm Reduction, Critical Time Intervention, and Housing First.
Qualifications:
Knowledge of evidence-based interventions (e.g., Critical Time Intervention, Harm Reduction, Crisis Intervention, Motivational Interviewing).
Proficiency with Microsoft Office and database systems.
Respectful, non-judgmental approach with clients with complex needs.
Professionalism, motivation, adaptability, and ability to work in a fast-paced environment.
Preferred: Active licensure with the California Board of Behavioral Sciences (e.g., LCSW, ACSW).
Minimum:
Master's Degree in Social Service or Clinical field.
At least 1 year of experience in healthcare, case management, or working with vulnerable populations.
Mandatory Requirements:
Flexible hours, including evenings/weekends.
Employment eligibility verification.
CPR/First Aid certification (or willingness to obtain).
Proof of COVID-19 vaccination.
Complete Tuberculosis, Background, and Drug Tests.
Driving: Reliable transportation, valid CA driver’s license, proof of insurance, and ability to qualify for PATH's insurance coverage. Driving is an essential function.
Employment Eligibility: Must be authorized to work in the U.S. without sponsorship. No current services from PATH or recent past services are permitted.
Work Location: All work performed within California. Hybrid schedules may be available based on responsibilities.
