POSITION SUMMARY:
Clinical Case Managers provide comprehensive psychological and behavioral case management services within a continuum of high to low intensity services designed to improve or maintain each person we serve’s abilities to function in the community. They assess, monitor, and follow up with client needs toward recovery, and are responsible for all clinical documentation of consumer interaction. The Clinical Case Manager will also provide transportation for clients in their vehicle (additional cost for insurance reimbursed).

ESSENTIAL FUNCTIONS:
Help clients meet basic needs for food, clothing, shelter, personal safety, and general medical and dental care, and assist with applications for income, medical, housing, or other benefits.
Identify clients who require a protective payee‑ship and arrange this assistance.
Ensure clients have access to needed services and community resources by arranging transportation, or by taking them to services in the Clinical Case Manager’s vehicle.
Provide comprehensive psychosocial services, including a continuum of high and low intensity services designed to improve or maintain clients’ abilities to function in normal social roles. These include:
Helping clients evaluate strengths and symptoms, and facilitating goal setting and appropriate services planning.
Guiding and instructing clients in daily living skills such as medication use, diet, exercise, personal hygiene, shopping, cooking, budgeting, housekeeping, transportation, and community services.
Helping clients develop social skills, interests, and leisure activities to promote participation and self‑worth, including age‑appropriate, culturally appropriate daytime and evening activities.
Assisting clients in finding and using appropriate employment opportunities, vocational rehabilitation services, or supported work environments.
Supporting clients who need special living arrangements in locating rehabilitative or supportive housing that provides the necessary support while encouraging increased personal responsibility.
Offering support, assistance, consultation, and education to families, friends, landlords, employers, and community agencies to maximize benefits and minimize problems associated with clients’ community presence.
Identify and collaborate with natural support systems such as neighborhood networks, churches, and community organizations.
Encourage treatment program engagement by fully utilizing clients’ support systems.
Reach out to program‑eligible clients, inform them of, and educate them about available services at WellPower and in the community.
Maintain a trauma‑informed environment of well‑being.
Perform other duties as assigned.
REQUIRED QUALIFICATIONS
Education
Bachelor (CM I) or Master (CM II) degree in psychology, social work, or another related human services field.
Experience
One year of experience in mental health or human services preferred.
Valid Colorado driver’s license and a recent clean driving record; must be able to transport clients in own vehicle.
Ability to obtain and carry business‑use coverage on auto policy (additional cost reimbursed).
TYPICAL PHYSICAL AND MENTAL DEMANDS
Ability to sit, stand, bend, reach, drive a vehicle, and operate standard office equipment such as phone, computer, fax, and copier. Occasional lifting up to 40 lbs.
