Pay Range & Benefits
CM I – $28.96/hr (≈ $60,237 annually) – Bachelor’s degree required
CM II – $30.05/hr (≈ $62,504 annually) – Master’s degree required
Language Differential: up to $2.50/hr

Position Summary
As a Clinical Case Manager, you will deliver comprehensive psychological and behavioral case management services across a spectrum of intensity levels. Your responsibilities include assessing clients’ basic needs, coordinating transportation and services, monitoring progress, and providing psychosocial interventions to improve clients’ functional abilities.
Essential Functions
Help clients meet basic needs for food, clothing, shelter, personal safety, and medical/dental care, and assist with applications for income and benefits.
Identify clients who require a protective payee and arrange assistance.
Arrange or provide transportation to service locations, or take clients in your personal vehicle when necessary.
Provide comprehensive psychosocial services across a continuum of high‑ to low‑intensity interventions to improve clients’ functional status.
Design and facilitate goal‑setting and skills training in daily living, medication management, diet, exercise, hygiene, shopping, budgeting, and transportation.
Assist clients in developing social skills, interests, and leisure activities; facilitate employment and vocational rehabilitation opportunities.
Support clients who need specialized housing arrangements, locating appropriate rehabilitative or supportive housing.
Collaborate with natural support systems such as neighborhood networks, churches, and community organizations.
Maintain a trauma‑informed environment and promote treatment engagement.
Other duties as assigned.
Required Qualifications
Education: Bachelor’s degree in psychology, social work, or related human services degree for CM I; Master’s degree for CM II.
Experience: One year in mental health or human services preferred.
A valid Colorado driver’s license with a clean driving record.
Proficiency in transporting clients in your personal vehicle; ability to obtain and maintain business‑use auto coverage if required.
Physical and Mental Demands
Ability to sit, stand, bend, and reach; drive a vehicle; operate standard office equipment; occasional lifting up to 40 lbs.
