Schedule: Hybrid, Monday to Friday (8:30am-5:00pm)

Base Pay Range: $19.79/yr – $24.80/yr

Summary
This position provides a broad range of specialized clinical case management services within guidelines established by the Organization and specific programs. The role functions with a high degree of independent accountability in accordance with professional and ethical standards, organizational policies, and specific contractual requirements. Populations served may include children, adults, families, immigrants, refugees, and individuals with mental illness, HIV/AIDS, and/or substance abuse issues. Individual accountabilities and work volume will be established through the development of annual Success Objectives within the framework outlined below.

E​ssential Functions

Provide clinical case management services to program participants through a short‑term case management model.

Receive case assignments, establish service plans, review case progress, and determine case closure in accordance with funder requirements.

Meet regularly with assigned cases and representatives from the community as needed.

Provide individual counseling to address motivational and psychosocial issues of program participants.

Perform intense advocacy, crisis intervention, counseling, and problem solving in a variety of areas for all program participants.

Conduct outreach and intake as requested.

Assist program participants in increasing or restoring their self‑sufficiency.

Develop service plans under supervisory guidance.

Make referrals in accordance with service plans to other organizational units and external providers.

Screen for income supports benefits and entitlements to advocate on behalf of program participants.

Implement and monitor treatment plans to ensure realistic goals and deadlines are met.

Provide financial assistance and financial‑management support in accordance with organizational policies.

Complete all necessary forms, case recordings, and statistical reports accurately and promptly.

Develop and maintain professional relationships with social service, health, and governmental agencies.

Participate in supervision meetings, team meetings, in‑house training sessions, conferences, and operational planning.

Carry out other tasks as assigned.

Education & Experience

BA/BSW and three years’ experience or master’s degree in social work, counseling, clinical psychology, or related field.

One year of experience in case management, counseling, clinical psychology, or related field.

Certifications & Licenses
A valid driver’s license may be required.

Knowledge/Skills/Abilities (K/S/A)

Read and interpret documents such as operating instructions and procedure manuals.

Write routine reports and correspondence.

Speak effectively before groups of customers or employees.

Have reliable transportation to main and scattered sites.

Work effectively with people from varying socioeconomic backgrounds.

Provide trauma‑informed care with a patient‑centered approach.

Perform basic arithmetic and calculate rates, ratios, and percentages.

Excellent communication skills and strong relationship building.

Handle confidential information professionally and diplomatically.

Exhibit collaboration, emotional intelligence, self‑motivation, and analytical ability.

Proficiency in Microsoft Office, Internet Explorer, Outlook/Exchange, Salesforce, and other software used by Heartland.

Apply practical problem‑solving skills in variable situations.

Interpret instructions in various formats.

Work independently with minimal supervision.

Strategic thought partnership and consultancy skills.

Work Arrangement

Eligible for a hybrid arrangement: two days on site per week and in‑person meetings when required by location.

Employees may need to conduct home visits at participants’ residences across Chicago.

Work Environment

Moderate noise level.

Program sites throughout Chicago and surrounding suburbs.

Possible temporary reporting to any location.

Physical Demands

Regular talking and hearing.

Regular sitting, standing, and walking.

Stoop, kneel, and/or crawl as needed.

Use hands for typing, handling, or reaching.

Occasional lifting or moving up to 25 pounds.

Require close, distance, color, peripheral, and depth vision.
