Clinical Care Transition Specialist / Registered Nurse - Central Valley
Job Summary: The Clinical Transition Specialist is an experienced sales professional who uses sales techniques to sell Option Care products and services to discharging patients. They are responsible for educating patients, their families and the facility staff about how the services and products will be facilitated at an alternative site.
Clinical Transition Specialists ensure proper placement of patients within the Home Health Care setting by assessing patients, gathering preadmission information, collaborating with internal (intake) and external (case managers, discharge planners) partners to ensure quality of service and implementation of an effective treatment plan. They proactively build relationships with referral sources and partner with Account Managers to grow referral rates and achieve sales goals.
Responsibilities
Proactively maintains and grows relationships with referral sources to increase sales and patient starts; serves as key point of contact and representative of Option Care to provide education, assistance, and service.
Interacts with area service providers to sell Option Care services and assess patient candidacy for alternative site care.
Educates patients, families and facility staff about services and products.
Provides in-person education to patients and caregivers to support timely discharge and therapy independence.
Communicates with agency staff, medical team, patients and family to implement an effective treatment plan.
Collects, reviews and completes pre-admission information and secures related signoffs.
Partners with Account Manager and Regional Sales Director to create and execute area business and growth plans.
Maintains ongoing communication with Account Manager to discuss opportunities and progress.
Collaborates with marketing to deliver educational and promotional programs to patients, caregivers and referral sources.
Reviews medical records to obtain pertinent history and insurance information; communicates with patient registration.
Coordinates care transition with referral sources and internal partners to ensure seamless transitions to home or alternate site; supports data collection for therapy start and patient tracking; facilitates transfer of orders.
Maintains confidentiality and adheres to legal guidelines regarding patient and company information.
Oversees and provides input to providers regarding patient assessment and transition to home care.
Acts as a point of contact and coordination with other providers; arranges for any special medical supplies or appliances as needed.
Qualifications
Licensed Registered Nurse (RN) or licensed Pharmacist in the state of practice with at least 2 years of healthcare experience.
Experience establishing relationships at all organizational levels and with vendors.
Knowledge of standard practices for services offered and relevant standards (e.g., ACHC, URAC).
Customer service experience with internal and external customers; ability to meet quality standards and evaluate satisfaction.
Basic PC skills: internet, email, Microsoft Word, Excel, PowerPoint.
Ability to identify operational issues and implement process improvements.
Reliable transportation to travel to care facilities, home visits and multiple hospitals; valid driver's license and proof of insurance if using a personal vehicle.
Ability to plan, organize and make presentations.
Travel
Travel to and from partnership facilities, community hospitals, and medical practice offices to sell Option Care services, process referrals and provide live education and training support.
