Program Description
Felton Institute’s Early Psychosis award winning program (re)MIND® (formerly PREP) was originally developed in partnership with the University of California San Francisco (UCSF). Felton’s (re)MIND® is an early psychosis coordinated specialty care model for early detection and treatment of schizophrenia spectrum disorders. (re)MIND® core evidence-based practices include Cognitive Behavioral Therapy for Psychosis (CBTp), Strength-Based Care Management, Family and Peer Support, Medication Management and Individual Placement and Support (IPS) model of supported education and employment. The (re)MIND® model has three additional components, Felton BEAM, BEAM UP, and (re)MIND Alumni, respectively for individuals diagnosed with bipolar or mood disorders with psychosis, for individuals at clinical high risk of developing psychosis, and for early psychosis aftercare services. Funded through partnerships with county behavioral health departments, Felton's Early Psychosis Division operates outpatient coordinated specialty care for early psychosis programs in San Francisco, Alameda, San Mateo, and Marin counties, as well as the innovative (re)MIND® Central program, which expands access to early psychosis coordinated specialty care.
Job Description
The (re)MIND® Central Clinical Care Manager I is part of a multidisciplinary team providing diagnostic assessment, treatment, and care coordination for individuals experiencing early psychosis. Responsibilities include evaluating client strengths and needs, developing collaborative treatment plans, and delivering therapeutic interventions addressing psychosis, mood, and co-occurring issues such as trauma, substance use, and environmental stressors. Services are provided in office, community, and telehealth settings. The Clinical Care Manager I maintains accurate, timely documentation in the electronic health record and reports to the (re)MIND® Clinical Program Manager (or as assigned by the Division Director). This position may require local travel between sites to better meet the needs of individuals in care.
Specific Responsibilities
Meet regularly with clients and their families at clinic sites, community locations, and telehealth (when appropriate).
Conduct assessment interviews and diagnostic evaluations with clients and parents to develop treatment plans.
Provide evidence-based treatment intervention (CBT for Psychosis), family psychoeducation, case management, risk assessment, and crisis intervention.
Collaborate closely with community providers, including making referrals to other services and supports when indicated.
Maintain clinical records after each client contact or service, in accordance with mandated quality assurance standards for MediCal, MediCare and other insurance, as well as in compliance with County and State documentation standards.
Meet all deadlines and comply with agency's documentation requirements and standards for required direct service hours.
Attend and contribute to team meetings and case conferences, providing client updates, discussing clinical issues and exchanging ideas.
Attend all scheduled trainings and follow requirements to maintain competency in evidence-based practices, as indicated.
Engage in outreach events, including psychoeducational presentations, open houses, and provide information about (re)MIND to other mental health providers and general public as needed.
Perform administrative tasks (i.e. chart completion, documentation, etc.), answer intake and referral calls, and perform other duties as assigned.
Minimum Qualifications
Two years of experience providing direct mental health services to youth and young adults with serious mental health challenges required. Direct service experience in an early psychosis program preferred.
Master’s degree (or higher) in Social Work, Counseling Psychology, Marriage and Family Therapy or Doctorate degree in Psychology or related and valid CA BBS or BOP license (Clinical Care Manager II). Associate-level applicants (ASW, AMFT, APCC) may be considered (Clinical Care Manager I).
Familiarity with research-validated interventions and demonstrated readiness for advanced professional development and training.
Experience with public and private mental health services, community resources and TAY client population preferred.
Ability to provide services in a variety of settings and excellent professional skills for time management, use of technology (EHRs, MS Office 365, cloud-based storage, video conferencing platforms).
Knowledge of State and Federal requirements for clinical documentation and reporting, including MediCal documentation standards.
Strong clinical assessment skills (DSM-5 diagnoses), well-developed interviewing skills, and working knowledge of structured diagnostic assessments such as SCID-5, NetSCID, SIPS.
Bi-lingual Spanish preferred but not required.

Salary Range: $100,000 - $130,000 Annually (DOE)
