JOB
SUMMARY:
The Clinical Care Coordinator (C3)
proactively identifies and removes barriers that may impede department or system
wide patient flow by serving as a consultant and educator with medical staff
regarding patient status, bed placement, payor requirements, and utilization of
resources. The position (Unit C3) will
act as the primary liaison between emergency and surgical services, nursing,
behavioral health, finance, physicians, utilization review to ensure
appropriate bed assignment, patient care needs are met, and treatment plans
are executed at the appropriate levels. The role involves working collaboratively with all members of the healthcare team to improve patient throughput and length of stay (LOS) performance of the organization.
JOB
QUALIFICATIONS:
Bachelor's
Degree in a health-related field is required.

Three
(3) or more years of clinical experience are required.

Nurse
Case Management experience is preferred.

Registered
Nurse in the State of Georgia is required.

Basic
Life Support (BLS) Certification is required.

For
Critical Care and ECC units, Advanced Cardiac Life Support (ACLS) is required.

Certified
Case Manager (CCM) or American Case Manager (ACM) Certification is preferred.
