Clinical Care Coordinator - RN, LGPC, LGSW, LPC, LICSW
Pay Range: $70,000-$80,000 (varies by credentials/education)
Job Summary
The Clinical Care Coordinator adopts a "whole-person" approach to address client needs related to physical health, behavioral health, and social determinants of health. CCC involves coordination of care between the behavioral health clinician and external providers (primary care, other behavioral health providers, hospitals). The Coordinator facilitates treatment planning and plan of care implementation activities separate from diagnostic assessment. The Coordinator ensures clients are at the appropriate level of care and, if progress is insufficient, facilitates transfer to a more appropriate level of care.
Summary of Essential Job Functions
Facilitate outreach to establish referral providers and participate in outreach events
Facilitate intake admissions and reassessments as scheduled
Maintain board‑approved and active licensure in the District of Columbia (credentials listed in the job description)
Prompt and regular attendance at an assigned work location
Complete assigned tasks/projects in a timely manner
Carry a caseload and determine the frequency of ongoing Diagnostic Assessment and Plan of Care services
Collect available social, educational, economic, and vocational information for assessment
Implement or revise patient treatment plans and record progress according to BHG policy
Develop formal treatment plans within 7 days of entry or as required by state guidelines
Maintain complete, comprehensive, and well‑organized patient records
Submit records for periodic review as requested by the supervisory team
Document all patient contacts; ensure documentation by other team members is recorded in the patient’s chart
Make appropriate referrals through case‑management services and perform necessary follow‑up
Assist the Clinical Director in implementing new programs into patient treatment
Advise the Clinical Director of problems encountered with caseload management and schedules
Attend weekly treatment team meetings and prepare to discuss individual patient caseloads
Maintain basic understanding of alcohol/drug abuse and addiction; keep up‑dated on current trends in Opioid Maintenance Treatment
Provide intake/readmission services and supportive follow‑up counseling for new patients until assignment to a primary counselor
Provide follow‑up calls/contact to all discharging patients for at least 30 days
Provide crisis intervention/support services to patients assigned to other counselors when their primary counselor is unavailable
Provide inter‑agency reports/documentation required for third‑party reimbursement, criminal justice proceedings, supplemental income assessments, emergency services, and other specialized treatment needs
Coordinate care with other community providers as needed
Other duties as assigned by supervisory team
Regulatory
Ensure compliance with all federal, state, and local regulatory bodies and accrediting agencies
Meet BHG attendance standards and be present and ready for work at the beginning of the established work schedule
Report any work‑related injury or illness, workplace hazards, or security issues immediately to the supervisor

Participate in community and public relations activities as assigned

Stay up‑to‑date with the latest software
Minimum Requirements And Qualifications
The Clinical Care Coordinator must hold one or multiple of the following licensures: Physicians, psychologists, LICSW, LGSW, APRN, RN, LISW, LPC, LMFT, and LGPC. Must also have the appropriate experience for the role.

Organize time and responsibilities to meet workload requirements
Demonstrate basic knowledge and skill of office equipment and software
Physical Requirements And Working Conditions
Speaking and hearing ability sufficient for effective communication by phone or in person
Vision adequate to read correspondence, computer screen, forms, etc.
Prolonged sitting, some bending, stooping, or stretching
Variable workload, periodic high stress and activity level
Standard medical office conditions and environments
Interactions with patients who may be ill, have infectious diseases, have mental health diagnoses, and/or are involved in the criminal justice system
Keyboarding for long periods of time
