Overview Clinical Care Coordinator, FOH, Northampton — CSO is looking for a Clinical Care Coordinator for our team at Friends of the Homeless in Northampton.
Responsibilities Complete comprehensive biopsychosocial assessment
Participate in early and ongoing team collaboration with case managers and outreach workers to ensure program participants' needs are identified and coordinated
Develop and implement an individual service plan (ISP) in collaboration with the individual collaterals
Develop a risk management/safety plan in collaboration with the individual and collaterals
Facilitate care planning team (CPT) meetings
Identify and develop natural supports
Provide substance abuse and behavioral health education, advocacy and support
Provide therapeutic support and services for uninsured and underinsured individuals
Provide on-site psychoeducational groups related to specific BH/SA diagnoses and issues
Your Background Master\'s degree in a behavioral health discipline or related field (required)
Clinical license (preferred)
Valid driver\'s license and reliable transportation required
