Minimum Qualifications (MUST-HAVE) These qualifications are essential for success in this role:

  • 5+ Years of Home Care/Case Management Experience: Must have five or more years of experience directly in home care, case management, or client care coordination within a health care setting.
  • Case Management Expertise: Proven ability to perform comprehensive client assessments, develop, and manage complex care plans, and effectively coordinate services.
  • Leadership/Supervisory Experience: Prior experience in supervising or mentoring caregiving teams and leading client/family introductions.
  • Required Licensing: Current driver’s license and proof of auto insurance.

Key Responsibilities

The Client Care Manager oversees all aspects of client engagement and service delivery.

  • Conduct in-home quality care meetings and develop comprehensive care plans tailored to client needs.
  • Maintain ongoing communication with clients, families, caregivers, and referral partners.
  • Monitor client satisfaction and implement service adjustments as needed to ensure quality outcomes.
  • Partner with the scheduling and care teams to align caregiver skills with client preferences and requirements.
  • Identify opportunities for growth through relationship-building and client retention initiatives.
  • Accurately document all client interactions and updates in accordance with agency standards.
Preferred Qualifications
  • Bachelor’s degree in health care administration, nursing, social work, gerontology, or a related field.
  • Experience with WellSky/ClearCare scheduling software.
  • Strong organizational and time management skills, with attention to detail in documentation and follow-up.
