Overview: The Care Manager plays a key role in ensuring quality, compassionate, and client-centered care for individuals receiving non-medical home care services. This role involves coordinating care plans, maintaining compliance with Nevada home care regulations (NAC 449), and acting as a liaison between clients, caregivers, and administrative staff.
Key Responsibilities: • Conduct initial in-home assessments to determine client needs and develop personalized care plans • Match clients with qualified caregivers based on needs, preferences, and scheduling • Provide ongoing oversight and support to both clients and caregivers, including routine check-ins • Monitor and update care plans as needed based on client condition or feedback • Ensure services remain compliant with Nevada non-medical home care standards and agency policies • Maintain accurate client records and documentation in accordance with HIPAA and state requirements • Address client concerns or incidents promptly and professionally • Collaborate with scheduling and HR departments to ensure proper staffing coverage • Educate clients and families on available services and set expectations clearly
Qualifications: • Must have at least 1 year of experience working in healthcare or Homecare • Must have a bachelor’s degree in business or an Associate’s degree in a related field • Prior experience in home care, case management, or a similar role strongly preferred • Strong communication, organizational, and problem-solving skills • Knowledge of Nevada home care laws and regulations • Ability to work independently and handle sensitive information with discretion • Bilingual (Spanish/English) a plus
Additional Information: This is a non-clinical role. The Care Manager does not provide hands-on medical care, but focuses on service coordination, quality assurance, and ensuring compliance within the scope of non-medical home care under Nevada state law.
