Client Care Coordinator (Park Overlook)
PRIMARY FUNCTION/PURPOSE:
The Client Care Coordinator is responsible for the overall delivery and coordination of mental health and related services for homeless individuals. The Client Care Coordinator will enhance existing services to include a thorough assessment of clients’ long-term goals at intake, and collaborate with the case management staff to set achievable goals while establishing linkages within the community to ensure their successful progress towards independent living. This position will work with individuals at our Park Overlook location in the Bronx, NY.
