Chronic Care Manager (Remote – Compact States)
Join Harris Computer’s Chronic Care Management team as a telephonic encounters provider for patients with two or more chronic conditions. This role is a 1099 independent contractor position with the potential to transition to full‑time W2 employment and benefits.

Location & Schedule

Remote – home office in the United States

Part‑time: minimum 20 hours per week, flexible hours and no daily schedule

Minimum patient panel: 100 patients per month, required 90 % encounter completion

Compensation

Productivity‑based pay structure (see tiers below)

$10.00 per encounter for 0‑99 encounters/month

$10.25 per encounter for 100‑149 encounters/month

$12.00 per encounter for 150‑199 encounters/month

$14.00 per encounter for 200‑249 encounters/month

$16.00 per encounter for >250 encounters/month

Payment tier increases require 3 months consistency

Each encounter takes a minimum of 20 minutes

Responsibilities

Perform telephonic encounters on behalf of partner practices each month

Develop and document detailed care plans using the electronic health record templates

Abide by the plan of care and orders of the practice

Provide prevention and intervention for multiple disease conditions through motivational coaching

Improve practice revenue by creating billable CCM episodes and increasing visits for chronic‑condition management

Attend scheduled staff meetings and maintain accurate documentation in the EHR

Use Windows desktop applications (Office 365, Teams, Excel) in a HIPAA‑compliant home environment

Qualifications

Graduate from an accredited School of Nursing (LPN, LVN, RN, BSN, etc.)

Current compact state RN/LVN/LPN license with no disciplinary actions

Minimum two (2) years of clinical experience in Med/Surg, case management, or home health

Hands‑on experience with electronic medical records and Windows desktop applications

Strong organizational, time‑management, problem‑solving, and decision‑making skills

Excellent verbal and written communication; high‑speed internet and Windows/Mac desktop or laptop (no Chromebook)

Self‑directed with the ability to prioritize responsibilities and demonstrate phone etiquette
