The Child Case Manager is responsible for providing high-quality, person-centered case management services from intake through discharge or transition. This role supports individuals with mental illness, intellectual disabilities, or substance use disorders in achieving improved health, stability, and independence. Duties include coordinating services, maintaining documentation, and collaborating with a multidisciplinary team to ensure persons served receive timely, effective, and compassionate care.

Qualifications and Education Minimum Requirements
Applicant(s) must be qualified as a CMHP with 3 years experience with a bachelor's degree or 1 year experience with a master's degree in the examination, evaluation, and treatment of minors and their families. State license as an LMSW, LLMSW, LMFT, LLMFT, LBSW, LLBSW, LPC, LLPC, TLLP, LLP, LP, RN, or a degree in a human service field from an accredited school. Human service fields can include, but not limited to Human Services, Psychology, Sociology, Criminal Justice, or Social Work.
Must have a valid Michigan driver’s license, current automobile insurance, and the use of the insured vehicle during work hours.
