Case Management Professional – Washburn Center for Children
Washburn Center for Children is a leading children’s mental health center serving nearly 4,800 children and 11,000 families each year. This role supports children 3–17 through therapeutic care in homes, schools, and clinics.

Role Responsibilities

Manage a caseload of 17–18 children or youth and their families.

Provide culturally based, individualized case management in homes, schools, and community settings.

Coordinate mental‑health and other health‑related services, including insurance and medical assistance.

Complete timely documentation: Individual Family Community Support Plans, progress notes, and case‑management authorizations.

Educate families to empower self‑advocacy and navigation of systems.

Day‑to‑Day Activities

Assessment: Conduct functional assessments of medical, social, educational, and other services needed.

Service Plan Development: Develop Individual Family Community Support Plans (IFCSP) based on assessments and diagnoses.

Service Coordination: Refer, coordinate, and monitor services across providers, ensuring Minnesota and HIPAA data‑privacy compliance.

Education and Experience Required

Bachelor’s degree in social work, psychology, human services, sociology, or related field.

Licensure: LSW (License of Social Work) required; BSW acceptable.

1 year of experience with mentally health‑compromised children and families.

Fluency in English and Spanish.

Valid driver’s license and reliable transportation.

Knowledge of cultural responsiveness and the role of culture in therapy.

Salary
Salary Bracket: $46,000 – $50,000 (based on licensure, experience, and education).
