Position Summary
The DSCC Core / Connect Care Nursing Consultant provides care coordination services to families eligible for two programs. Under the regional manager and assistant directors, the consultant engages and develops strong partnerships with families through comprehensive assessments and person‑centered care plans, monthly interactions, and resource coordination. The role also offers consultation to the multi‑disciplinary team using RN skills and knowledge.

Responsibilities

Perform active care coordination services, completing comprehensive health assessments, identifying strengths, and developing person-centered service plans.

Facilitate 30‑day (or as needed) monitoring of the care plan, assessing status changes, prioritizing unmet needs and resource location.

Utilize a culturally competent approach, supporting families’ cultural values and traditions, and providing interpreter services or accommodations per ADA guidelines.

Participate in Medicaid managed care clinical rounds and DSCC multidisciplinary meetings as needed, engaging with the transition of care team for effective discharge planning.

Conduct and document in‑person home visits (every 6 months or as needed) or in other settings, ensuring timely documentation within 48 hours to maintain case record compliance.

Collaborate with providers to resolve critical incidents, managing clinically complex caseload participants, and applying communication skills to improve health literacy.

Arrange, lead, and contribute to interdisciplinary care team meetings, escalating complex behavioral/social determinants or clinical factors impacting participants’ well‑being.

Contribute to quality improvement initiatives, attend quality huddles, and provide recommendations.

Assist families and caregivers with coordination of medical services, treatments, supplies, equipment, and environmental modifications.

Mentor/coach care coordination team members and participants/caregivers on self‑management, medication adherence, and prevention.

Serve as a subject matter expert on health education initiatives such as immunizations, weight management, and physical activity.

Support other coordinators in verifying and interpreting clinical conditions, tailoring person-centered care plan priorities, and recommending resources.

Assist families with private/public health insurance benefits management practices.

Perform other related duties as assigned.

Minimum Qualifications

Registered Nurse licensed in Illinois (or qualified for licensure within five months if from another state).

Bachelor’s degree.

At least two years of public health or specialized nursing experience.

Preferred Qualifications

Experience in care coordination, Medicaid managed care, or interdisciplinary team leadership.
