What You’ll Do
Deliver skilled, compassionate nursing care to patients in their homes
Conduct holistic assessments—physical, emotional, spiritual, and environmental
Collaborate with patients and families to create individualized care plans
Provide ongoing evaluation and adjust plans as needs evolve
Coordinate with the Hospice Director of Nursing to ensure seamless 24/7 coverage
Participate in on‑call rotation (with reduced frequency as census grows)
What You’ll Bring
Current RN license in the State of Wisconsin
At least 1 year of experience in hospice, home health, med/surg, or skilled nursing preferred
Exceptional communication and problem‑solving skills
A compassionate bedside manner and dedication to patient‑centered care
Valid driver’s license and current auto insurance
